    We are seeking a Clinical Manager Registered Nurse to join our team in Tampa, FL.   Pinnacle Home Care is a full service Medicare Certified and Florida Licensed Home Health Company locally owned and operated by home health professionals who are passionate about delivering exceptional, quality care to the communities that we serve.

    Responsibilities

    Duties of position:

    1. Coordinates all patient care services.
    • Case Conference – reviews and assists with the case management of active patients.
    • Runs “oasis and visits in status” reports daily and follows up with field clinicians as necessary to assist with timely and proper documentation.
    • Manage HHRG scores and clinician utilization to optimize fiscal integrity and prevent re-hospitalizations.
    • Review all interim orders in EMR system daily and follow up with clinicians as necessary. Verify any changes in visit frequency orders match the schedule and review episode utilization at that time (all disciplines).
    • Manage and receive report daily by field clinicians to review oasis assessments and visit frequencies to assure proper utilization and prevent. re-hospitalizations. Verifies visit frequencies are plotted properly on patient schedule (up to and including d/c or recert). Verify ordering physician at time of report and manage and follow up on physician face to face encounters.
    1. Reviews, accepts and coordinates referrals.
    2. Directly supervises and manages Administrative Clinical Assistants, including but not limited to oversight of patient and staff assignments.
    3. Directly supervises field clinicians, including annual reviews and on site competency visits. Also includes oversight of compliance with timely submission of all documentation.
    4. Facilitates reporting to physicians and communication with and between disciplines. Ensures communication with all physicians involved in the patient’s plan of care and the integration of all orders from all physicians involved in the plan of care to assure coordination of all services.
    5. Follows up on all laboratory and diagnostic tests – notifying both physician and field clinician of results.
    6. Documents in the EMR communications and interim orders as received. Includes goals with measurable time frames in interim orders. Manages patient census, lupas, outliers, and all services for fiscal integrity.
    7. Participate and manage areas of QIRT review process.
    8. Ensures proper maintenance of medical records as prescribed by – and in compliance with – local, state, and federal laws.
    9. Ensures that all orders are current and being followed precisely by clinicians.
    10. Assist with in-service education programs for all field staff and assists/holds staff meetings as necessary.
    11. Participates in the agency’s Quality Improvement program. Collaborates with QAPI and assists in the identification of and education of individual abilities and deficits in documentation.
    12. Responsible for ensuring that quality patient outcomes and team goals are within bench marked expectations.Assists field staff with discharge planning at time of start of care throughout the episode of care, ensuring progress towards goals.
    13. Demonstrates in depth knowledge of, and ensures compliance with, all local and state, and federal laws relating to all the operations of the agency (including office operations, way and hour regulations, and equal opportunity employment).
    14. Monitors and oversees the Beneficiary Elected Transfer process to ensure 1) proper documentation, 2) proper consideration of patient’s rights, and 3) the following of Palmetto’s established rules and guidelines.
    15. Carries out other duties as assigned.
    16. Manages and follows up on electronic and manual faxes as required.
    17. Assists with customer phone calls as required following established Standard Operating Procedure.
    18. Manages and responds to agency email throughout the assigned shift.
    19. Awareness of the Agency’s QAPI program. Active participation as required per policies and procedures related to the Agency’s QAPI program.
    20. Management and follow up of customer complaints, following Agency’s complaint policy.
    21. Acute awareness of patient rights and the Agency’s responsibility in honoring patient’s rights. Awareness of patient rights description in Agency’s service agreement and Agency’s admission packet.
    22. Awareness of appropriate reasons for a patient discharge and Agency’s policy on appropriate reasons for discharge.
    23. Awareness of potential signs of abuse and neglect and the steps required to report suspected abuse and neglect.
    24. Awareness and ability to design and identify patient centered and individualized goals with realistic measurable time frames.
    25. Awareness of infection prevention and control techniques, both within their scope of practice and within the Agency’s policy and procedures. Awareness and participation in the Agency’s infection control program and the Director of Clinical Service’s oversight of this program, along with the QAPI designed program.
    26. Awareness of employee infection policies and procedures and the Director of Clinical Service’s oversight of the program, along with the QAPI designed program.
    27. Maintains good standing in health care community.
    28. Maintains patient and employee confidentiality.
    29. Assists Executive Management in the execution of Pinnacle’s Mission and Values in a manner that is consistent in practice and message among clinical and administrative personnel.
    30. Participates in hands on skilled care of patients as the need arises.
    31. Maintains patient and proprietary confidentiality.
    32. Promotes the Agency in a constructive manner at all times and cultivates positive working relationships.
    33. Assists with Additional Documentation Requests (ADR’s) as required.

    Qualifications

    Qualifications:

     

     

    Must be a graduate of an accredited school of nursing (Bachelor of Science in Nursing preferred) and be currently licensed in the state of Florida. Must have a minimum of three (3) years recent experience in a home care agency. Must have two (2) satisfactory references from previous employers, which must verify supervisory nursing experience. Must possess and maintain current certification in cardiopulmonary resuscitation. Must possess excellent administrative, management, and communication skills. Must be flexible regarding working hours and days; exhibit professional behavior and display good emotional health.
